Use this form to request access to, correction of, or erasure of your own personal app account data (email address, profile info, and photo), or to exercise another privacy right such as withdrawing consent. Please select your request type below. We are only able to process requests for data we control from your ID123 app account. If you need to exercise a right over data controlled by your institution-issued ID card, please contact your institution to complete your request. Do not fill out this form for someone other than yourself. Once you submit this form we will send you an email to verify and prove your identity. Once your identity is confirmed, we will complete your request. The information provided below will only be used to complete your request.

You can also delete your own app account data at any time by going into the settings of your ID123 app, selecting “My Data”, and then selecting the option to “Delete App Account”.
