Self Data Erasure Request
Use this form if you are an individual requesting the erasure of your own personal app account data (email address, profile info, and photo). If you need to erase the data controlled by your institution, please contact your institution to complete your request. We will request verification from both of you in a separate email.
Do not fill out this form for someone other than yourself.
Representative Data Erasure Request
Use this form if you are an authorized administrator at your institution to request the erasure of an individual’s personal app account data (email address, profile info, and photo). If you need to erase your institutions related to the ID cards you issued, you can erase that data from the ID Management System. We will request verification from both you and the individual in a separate email.
To use this form you have to be an authorized administrator at your institution.
