Use this form if you are an authorized administrator at your institution requesting access to, correction of, or erasure of an individual’s personal app account data (email address, profile info, and photo), or exercising another privacy right on their behalf. If you need to exercise a right over data related to the ID cards you issued, you can do that directly from the ID Management System. We will request verification from both you and the individual in a separate email.
